Gary Oftedahl: When is a house a home—from a health care perspective

Over the past year I have often addressed the evolving patient-centered medical home concept, and the many hopes and visions for what this might do in transforming the health care system. Yet in the flush of activity, there is often times significant evidence that we have not come to a shared perception or common purpose regarding what the ultimate product should look like when fully implemented at the front lines of the health care system.
